Adult performers complete and sign this form for themselves. For performers under 18, a parent or legal guardian must complete and sign it. Teachers may not complete or sign applications on behalf of performers.

Before registering, review the festival guidelines, general recital guidelines and dress code. Current dates, deadline, venue and fee are shown below from the branch calendar and payment product.

After submitting, follow the payment instructions and use the same email address at checkout. Fees are non-refundable. Recital times are assigned after registration.
